Preparation and support for infant feeding in pregnant women with diabetes in the West Midlands: Research Report
This study was undertaken against the background of low breastfeeding rates among diabetic women in the West Midlands (Shah, Brydon and Gardosi 2010) in order to gain information from women and practitioners about current antenatal services for women with pre-existing diabetes (Type 1 and 2) and how women's choices, preparation and support for infant feeding are addressed by those giving women care in pregnancy. In particular, we wanted to consider to what extent national guidelines (NICE 2008, 2008a) are reflected in the practice undertaken in maternity units in the West Midlands and to identify gaps in the implementation of best practice. In addition, the study sought to explore midwives' knowledge of and perspectives on the possibility of antenatal expression of colostrum as a contribution to breastfeeding support and early neonatal care, and to explore diabetic women's experiences of advice, care and support offered by professionals regarding infant feeding. It was anticipated that the study might identify innovative and/or exemplary practices which might inform future practice development.

The West Midlands pilot breastfeeding education programme
An independent qualitative evaluation of the West Midlands pilot of the Heath Behaviour Research Limited (HBR) breastfeeding training programme was conducted to gather in-depth data from users about their experience of completing the training, in order to inform DH West Midlands and NHS West Midlands and its partners about the delivery and implementation of the programme in practice. The programme was commissioned from Health Behaviour Research Limited based at Coventry University's Techno Centre.

West Midlands Breastfeeding Training and Capacity Building Project: Final Report
The West Midlands pilot of breastfeeding training was a collaborative, joint funded initiative between the Department of Health West Midlands and the NHS West Midlands West Midlands Public delivered by Health Behaviour Research Ltd.
The purpose of the project was to develop the breastfeeding knowledge and skills of healthcare professionals in maternity primary care and community settings in the West Midlands through a pilot assessment and training programme in 3 PCT healthcare economies in the region.
The West Midlands pilot set out to implement an innovative approach to providing training to whole healthcare economies by using a rigorous assessment method and a flexible self study training programme with individualised feedback to improve breastfeeding support by healthcare practitioners. Staffing in Maternity Units: Getting the right people in the right place at the right time
This report was commissioned by The King’s Fund to answer a fundamental question:
Can the safety of maternity services be improved by more effectively deploying existing staffing resources?
There is much debate at present about staffing levels in maternity. The independent inquiry into the safety of maternity services commissioned by The King’s Fund suggested that, while staffing levels are important, employing more staff may not necessarily improve safety. The inquiry found that the effective deployment of the right staff doing the right thing at the right time in the right place is the key to improvement (The King’s Fund 2008, p 48).
Current financial pressures mean that it is unrealistic to expect significant increases in numbers of staff. As such, maternity services – and the National Health Service (NHS) as a whole – will need to focus on developing new ways of working in order to maintain, and
increase, levels of safety and quality within the resources available.
This report considers the available evidence about the relationship between staffing levels and deployment practices and safety of care for mothers and babies. It focuses specifically
on the intrapartum period, which refers to labour and birth. In so doing, the report considers different staffing models and approaches. In recognition that current practice is likely to be more advanced than the published literature, a small number of case studies have been used to offer examples of innovative activities.

Building blocks for a better start in life
Following on from the success of Change4Life, Start4Life is a new campaign aiming to support pregnant women and parents of children up to two years, in establishing good feeding and activity habits from the outset. Evaluation Start4Life Training Sessions: Pilot Project West Midlands 2010
This pilot project was carried out across the West Midlands for the West Midlands Public Health Observatory on behalf of the Department of Health. Twelve Start4Life training sessions were held over six days in 6 different locations, including Walsall, Nuneaton, Lichfield, Birmingham, Wolverhampton and Redditch. Most areas where the training sessions took place are currently working towards UNICEF Baby Friendly accreditation.

Evidence on the long-term effects of breastfeeding: Systematic reviews and meta-analysis
The Department of Child and Adolescent Health, in collaboration with the epidemiology unit in the University of Pelotas, Brazil, conducted this systematic review and meta-analysis of studies to assess the association between breastfeeding and blood pressure, diabetes and related indicators, serum cholesterol, overweight and obesity, and intellectual performance. Two reviewers independently evaluated study quality, using a standardized protocol, and disagreement was resolved by consensus rating. Subjects who were breastfed experienced lower mean blood pressure and total cholesterol, as well as higher performance in intelligence tests. Prevalence of overweight/obesity and type-2 diabetes was lower among breastfed subjects. All effects were statistically significant but for some outcomes their magnitude was relatively modest.

Infant Feeding Survey 2005: A commentary on infant feeding practices in the UK
The Infant Feeding Survey has been conducted every five years since 1975 and captures a wealth of information about infant feeding practices in the United Kingdom. It helps to describe historical trends in infant feeding practice and identify opportunities for improved implementation of national policy. The Scientific Advisory Committee on Nutrition (SACN) Subgroup on Maternal and Child Nutrition (SMCN) has reviewed the findings of the 2005 Infant Feeding Survey. It particularly welcomes the significant increases observed in breastfeeding initiation across the UK and a marked reduction in the proportion of infants given solid foods at an inappropriately young age. This review recommends options to improve infant feeding practice and informs infant feeding policy. It also suggests areas for deeper exploration in future quinquennial surveys, particularly to take account of the national implementation of 'Healthy Start' in 2006 and to define its effects on practice.

The Baby Friendly Initiative
The Baby Friendly Initiative is a worldwide programme of the World Health Organization and UNICEF. It was established in 1992 to encourage maternity hospitals to implement the Ten Steps to Successful Breastfeeding and to practise in accordance with the International Code of Marketing of Breastmilk Substitutes.
